I did not have any expectations about visiting Dr Knightly as I have seen other professionals in the past and some were not very good; and others were mediocre. Very few were excellent. But he takes Medicare which is a huge benefit. Most do not. And I found him to be very professional; warm and engaging. Whether I will really feel differently or not remains to be seen. But the experience was overall a very good one and I would recommend him to anyone.

What Is Esketamine (Spravato)?

Leading Esketamine Therapy For Depression in Connecticut

A man in a white shirt is smiling for the camera
A black and white icon of a stamp with a heart on it.

Medically Reviewed By Dr. Thomas Knightly

Esketamine’s primary mechanism of action is via the glutamatergic system. It increases the levels of available glutamate, which is a neurotransmitter that plays a crucial role in mood regulation and synaptic plasticity. It is believed that esketamine can lead to the generation of new neural pathways leading to its rapid antidepressant effects.

Who is Spravato For?

Spravato has been approved by the FDA for treating adults with treatment-resistant depression, who have tried at least two other forms of oral antidepressants. Although ketamine has been researched as a treatment for anxiety, esketamine has not yet been examined for this purpose.

The Benefits of Spravato Therapy

The low dosage requirement of Spravato means that side effects are generally less common and more mild. Furthermore, unlike regular antidepressants, esketamine takes immediate effect - patients may notice a difference within a matter of hours, even regarding suicidal ideation.

At Evolve, we understand how difficult it can be trialing standard antidepressants, with possible complications when adjusting to prescribed medication and navigating side effects, all while waiting for the
medication to take effect. As such, we highly recommend Spravato for those who seek rapid relief.

Is Esketamine Different From Ketamine?

Esketamine is a molecule derived from ketamine, though it has different medical purposes. Ketamine’s primary medical function is as an anesthetic and for short-term sedation. Although Ketamine has been researched as a medication for treatment-resistant depression, it has not been FDA-approved, unlike esketamine.

Esketamine, which has received FDA approval, has been thoroughly researched in individuals experiencing suicidality and treatment-resistant depression. Unique to esketamine, the esketamine molecule can be absorbed through the nasal mucosa, making delivery of the drug less invasive in comparison to intravenous ketamine.¹

What Does Spravato Feel Like?

We appreciate that you will want to familiarize yourself with how Spravato is administered and feels.

Many patients report mild feelings of dissociation and disconnection from their bodies after receiving Spravato. This is a result of the anesthetic effect of ketamine. Though it may feel confusing or disorienting, it is not a painful experience. In addition to this, the sedative effect may lead to drowsiness, lasting up to a few hours after administration.

These side effects may be considered somewhat unique in comparison to other common medications. It is our priority to ensure your safety and comfort through professional supervision when Spravato is administered.

Will My Insurance Cover Spravato?

Most major insurance companies cover Spravato treatment, including commercial plans as well as Medicare.

"Dr. Knightly was without a doubt, one of the most professional, knowledgeable and experienced Psychiatrists I have ever seen. Honestly, it was an appointment I would look forward to, rather than just any generic Doctor visit, which we all definitely know is not where we want to be. His follow up and true commitment to his patients is hard to find these days. 5 Stars is not enough. He is better than any rating scale and I know will humbly continue to help as many people as he can."

"Dr Knightly is empathetic, professional & informative. He’s walked me through the process & has answered all my questions without hesitation, even the questions I’ve repeatedly asked. During my first session I was overwhelmed with emotion (I’ve tried many things to try to treat my depression) & he validated my experience & made me feel comfortable. I recommend anyone who is interested in TMS to visit his office, you‘re in good hands."
